Name            Description
HRM-1 Position Authorization Request Form
Use this form for approval to fill a vacancy, to create a new position, or to reclassify a position. See our Recruitment page for additional search information.
EMP-1 Employee Information Form
New employees and employees with a break in service of a year or more should complete this form as soon as an offer of employment has been accepted. This form collects all bio/demo data required for appointment processing.
HRM-2      Appointment Form
Use this form for all new appointments, including new hires and rehires (after a break in service), promotions/reclassifications, and internal hires (from a search).***The EMP-1 is required, in addition to the HRM-2, for all new employees and rehired employees with a break in service of a year or more).

VOL-2 Volunteer/Unpaid Appointment Form
Use this form to make all volunteer and non-employee appointments.
HRM-3 Change of Status Form
Use this form for all changes in status, including leaves and separations.  Do not use this form for reclassifications and promotions, which require the HRM-2 form (excluding faculty promotions).  See the directions for a list of change options.

HRM-4 Student Employment Transaction Form

Use this form for all GA/TA/RA and Fellowship appointments, terminations, and changes of status. Do not use this form for Student Assistant appointments (see Student Employment page).
UP-8A Request for Internal Extra Service

Use this form for all internal extra service appointments.  This is for UAlbany State employees performing extra service in their home departments or on campus, paid by UAlbany State funds.  This is not for Research Foundation employees or employees of other State agencies performing extra service at UAlbany (see RF extra service or NYS Dual Employment).
AC-1588 NYS Dual Employment Form

This form is required for employees of another SUNY or State agency who are performing extra service at UAlbany (State payroll); also required for all UAlbany (State) employees performing extra service at another SUNY or State agency.  ***See Extra Service policies.

EMP-1 Employee Information Form
New Employees and employees with a break in service of a year or more should complete this form as soon as an offer of employment has been accepted.  This form collects all bio/demo data required for appointment processing.
I-9  Employment Eligibility Verification Form
The Federal Immigration Reform and Control Act requires employers to verify the identity and employment eligibility of all new employees using the form I-9. On or before the first day of employment, the employee should be prepared to complete this form, and to personally present documents establishing identity and employment eligibility to the designated department official..

EACA Employee Access and Compliance Agreement
By signing the Employee Access and Compliance Agreement, the employee agrees to comply with the applicable laws and University policies and procedures governing the handling and use of campus business records.
 Oath [Statement in Lieu of] Oath of Office (for non-labor class employees)
New York State Civil Service Law §62 requires that professional employees take an oath of office, or make a statement in lieu of oath, upon original appointment or upon a new appointment after a break in service. Employees must certify that they have received and read the applicable sections of the New York State Public Officers Law, and that they will conform to the norms of conduct for State employees.

Affirmation Affirmation of Initial Service
The Office of the State Comptroller requires that each new employee certify their presence in person to the appointing authority’s designee on their first day of service at the University. 
AP-4 Professional Background Information Form
This form is only required of new employees if an updated CV or resume is not attached to the appointment packet.
Credentials Verification of Employee Credentials
The hiring department should verify the highest degree and all significant and relevant work experience of all newly hired academic and professional staff. A copy of the degree verification must be attached to this form (e.g. copy of diploma, copy of transcript, etc.)  This form should be signed by the department head attesting that verification has been completed.
W-4 Federal Tax Withholding Form

Direct Deposit You may choose to have your NET salary partially or entirely directly deposited via Electronic Funds Transfer with any financial institution within the 50 States that is capable of receiving transactions through the Automated Clearing House System.
